CPHS Dance Boosters Scholarship Process
CPHS Dance Boosters will award $4,000 in scholarships for the 2024-2025 school year:
● One Stacy Kaye Danielson scholarship for $1,800
● One scholarship for $1,200
● One scholarship for $1,000
Scholarship Review Committee
The scholarship review committee will be made up of an odd number (5-9) of members. Members of the booster club who have a child eligible for a scholarship may not serve on the scholarship review committee. Sponsor/Directors may not be involved in the selection process.
Scholarship process, application and committee members will be communicated by the first day of the school year on cphs.dance.

Submission Requirements
Download application from cphs.dance.
Applicants should submit the following documents via email to the CPHS Dance Booster Scholarship Review Committee Chair at scholarship@cphs.dance:
Fully completed scholarship application (submit in Word or PDF format).
Scanned copy of their current CPHS transcript.
Each applicant must be a graduating senior. Celebrities, Luminaries, Honor Guard, and managers are eligible for scholarships. Only one scholarship will be awarded to an applicant.
CPHS Dance Booster Scholarship Program will start accepting applications on March 9, 2025.
Scholarship Review Committee Chair will de-identify application packets. Application packets will be reviewed by the Scholarship Committee members and scored.
Deadline
CPHS Dance Booster Scholarship Program will start accepting applications on March 9, 2025.
Applications and supporting documents due by EOD (11:59pm) on April 6, 2025. Applications received after the deadline, or applications with incomplete or missing information, are not eligible for consideration.
Presentation
Recipients of the scholarships will be awarded certificates at the Spring Banquet on May 6, 2025.

Payment
The scholarship will be paid in one lump sum directly to the education institution after a letter of acceptance/confirmation of enrollment has been provided. Recipients have 6 months from the date of the awarding of the scholarship to request funds or communicate with the Scholarship Committee Chair and Treasure their educational plans.
● Scholarship committee chair will notify recipients that it is their responsibility to get the following to them:
○ Letter of acceptance/confirmation of enrollment
○ Department and address the scholarship payment should be mailed to
● Scholarship committee chair provides the treasurer the following:
○ A list of winners and amounts
○ The recipient’s education institution contact information
● The treasurer will submit scholarship award funds to the educational institution.
